Monogram Health  is  seeking a full-time Internal Medicine Physician in Mississippi.  Monogram actively manages patients with a wide range of acute and chronic conditions including CKD, and ESRD.  We work to improve access to healthcare and health outcomes for patients. We are committed to transforming the patient experience, especially for those who have been historically underserved. Monogram believes in utilizing technology to deliver efficient care. In our care model, we are increasing access, lowing costs, and driving better outcomes for patients with a wide range of chronic conditions.

This position is  not office-based . This is a remote (work-from-home) position.

Roles and Responsibilities :

  • Telehealth (Work from home), Monday-Friday 8:30 am-5:30 pm
  • Treat and diagnose patients with Polychronic as well as Kidney and Renal diseases
  • Serve as a collaborator, partner/mentor in the training of NPs and other providers to provide high-quality care
  • Apply evidence-based criteria in determining the appropriateness and necessity of care
  • Reduce unnecessary or preventable episodes of care, including hospital readmissions
  • Identify and make practical, pointed interventions to improve member care
  • Review Annual Wellness Visit assessments 
  • Conduct meaningful end-of-life and treatment options discussions with patients

Position Requirements :

  • 3+ years of Internal Medicine experience
  • Desire to practice in a population health management environment
  • Demonstrated experience applying evidence-based clinical criteria
  • Comfort and skill in advanced care planning
  • Experience integrating acute and behavioral health care
  • Expertise in managing medication panels for complex patients

Required Licensure:

  • Current residence and state medical license without restrictions to practice and free of sanctions from Medicaid or Medicare


  • Doctorate Degree in Medicine
  • Board Certified in Internal Medicine
