Monogram Health  is  seeking a full-time Internal Medicine Physician in Mississippi.  Monogram actively manages patients with a wide range of acute and chronic conditions including CKD, and ESRD.  We work to improve access to healthcare and health outcomes for patients. This position is  not office-based . This is a remote (work-from-home) position.

Roles and Responsibilities :

• Telehealth (Work from home), Monday-Friday 8:30 am-5:30 pm
• Treat and diagnose patients with Polychronic as well as Kidney and Renal diseases
• Serve as a collaborator, partner/mentor in the training of NPs and other providers to provide high-quality care
• Apply evidence-based criteria in determining the appropriateness and necessity of care
• Reduce unnecessary or preventable episodes of care, including hospital readmissions
• Identify and make practical, pointed interventions to improve member care
• Review Annual Wellness Visit assessments 
• Conduct meaningful end-of-life and treatment options discussions with patients

Position Requirements :

• 3+ years of Internal Medicine experience
• Desire to practice in a population health management environment
• Demonstrated experience applying evidence-based clinical criteria
• Comfort and skill in advanced care planning
• Experience integrating acute and behavioral health care
• Expertise in managing medication panels for complex patients

Required Licensure:

• Current residence and state medical license without restrictions to practice and free of sanctions from Medicaid or Medicare

Education:

• Doctorate Degree in Medicine
• Board Certified in Internal Medicine
